Our GP-led Urgent Care Centre based at Yateley Medical Centre guarantees local patents an "on the day" appointment for anything which is medically urgent. We are expanding our team of Nurse Practitioners who work alongside GPs to provide this service. Our Nurse Practitioners also play an important part in the management of chronic disease.

We offer a friendly & supportive environment for nurses to practice safely and effecively with a manageable and sustainable workload and a positive commitment to prefessional development.

We have roles which can take into account personal interests and skills and can offer a variety of working patterns to suit.

Diabetes Nurse Specialist (24 – 37.5 hours per week)

A fantastic opportunity has arisen for an experienced Diabetes Nurse Specialist who would like to take the lead on the provision of diabetic care to patients at our large GP surgery. We pride ourselves on being at the forefront of providing excellent patient care in our progressive and expanding GP Practice within our own Primary Care network in Yateley and Blackwater.

This is an exciting role which will enable you to use your specialist knowledge and skills and put you at the centre of how we manage our diabetic patients. You will be working as part of our dedicated and professional nursing team and amongst other responsibilities you will:-

  • act as diabetic nursing lead within the context of the multidisciplinary team throughout the patient journey
  • hold and manage an active case load and maintain responsibility for a defined patient caseload and to undertake patient assessment, develop, implement and evaluate plans of care for patients with all stages of diabetes
  • Review and prescribe diabetic treatment including injectable therapy, giving advice on up-to-date and evidence based management to patients and the wider clinical team
  • provide telephone support, assessment and advice to people with diabetes on their management in accordance with national and local guidelines and practice.
  • act as a resource for people with diabetes and their families, ensuring actions are consistent with clinical governance which may include devising and providing patient education
  • develop links with diabetic hospital services to improve integration of patient care.

A fantastic opportunity has arisen for an experienced (Chronic) Respiratory Nurse Specialist to join our nursing team in the provision of respiratory care to patients at our large GP surgery. We pride ourselves on being at the forefront of providing excellent patient care in our progressive and expanding GP Practice within our own Primary Care network in Yateley and Blackwater.

This is an exciting role which will enable you to use your specialist knowledge and skills to have a real positive impact on our nursing service for patients (both adult and paediatric patients) with chronic respiratory disease, including COPD and asthma. You will be working as part of our dedicated and professional nursing team and amongst other responsibilities you will:-

  • undertake spirometry and FeNo testing, and be able to interpret results in respect of diagnosis
  • hold and manage an active case load and maintain responsibility for a defined patient caseload
  • provide telephone support, assessment and advice to people with respiratory disease
  • act as a resource for people with respiratory disease and their families, ensuring actions are consistent with clinical governance which may include devising and providing patient education as well as assisting with end of life care
  • develop links with hospital and community respiratory services to improve integration of patient care, in addition to pharmaceutical suppliers

We will consider applications for both part-time and full-time hours. Applicants should be confident in diagnostics of respiratory disease and ideally hold an independent prescribing qualification.

As a large and progressive GP Practice based over three sites we are looking to recruit a Trainee Practice Nurse to work in our expanding nursing team. This is an ideal opportunity for someone with limited experience but with aspirations to progress their career within primary care.

We are a well-established practice where training and development is at the heart of what we do so that we can provide the highest quality care to patients. You will be working alongside a supportive and highly skilled clinical team and you will undertake a sustained training programme; Foundation in General Practice Nursing.
