There are disabled parking spaces close to the front door at all of our sites. Wheelchair access is available to all consulting rooms. We have accessible toilet facilities. A hearing loop is available at Reception.
A copy of the Practice Booklet with lots of information about the practice is available in large font - please ask Reception to arrange a copy if required.
Please make the Receptionist aware when you are booking your appointment if you are likely to need any additional assistance when you arrive at the surgery. For example, clinical staff will collect patients with sight impairment from the waiting room and accompany them to the consulting room if required.
We can arrange interpretation services in person or by phone for patients who do not speak English. A signing service is also available for patients who are deaf or who have limited hearing. Please let us know if you need one of these services when booking an appointment.
Midwives from Frimley Park Hospital are attached to the practice and share responsibility for ante and post natal care with the doctors. If you have done a positive pregnancy test, you do not have to see a doctor unless you have concerns. You should make a "booking in" appointment with the Midwives when you are about 8 or 9 weeks. The Midwives will then advise on all aspects of the pregnancy. Patients can, if they wish, contact the Midwives directly, to make the booking appointment themselves or speak to Reception.
The Midwives can be contacted by telephoning 01252 749598. All pregnant women will be given information on how to contact the Midwives in an emergency.
Clinics are usually held at Yateley Medical Centre and Monteagle Surgery.

Asthma, COPD, Spirometry
All patients should attend at least once a year for an annual review and more frequently if problems are experienced.
Available throughout the week on all sites with a Phlebotomist. Late appointments are available for those who work.
Cervical Smear Tests
Reminders are automatically sent out to women between 25 and 65 to book a test when due. Please call to book an appointment with a nurse.
Child Health Checks
The first baby examination is performed by the doctor at around 6 weeks from birth. Please ensure you make an appointment for this examination and do bring with you the baby’s ‘Red Book’.
Subsequent baby check-ups will be performed by the health visitors who run Baby Clinics locally.
Dedicated clinics are held at all sites for Pre School Boosters and Baby Immunisations. Other appointments are available on request. The first baby immunisations are due at the age of 8 weeks and baby needs to have had their 6 - 8 week check the nurse can give thier first vaccinations.
Child Health Clinics
These are drop-in clinics run by the local Health Visitors who can advise on the health and well-being of babies and children under 5.

Yateley Medical Centre (Thursday 13:00 - 14:30)
This test is free and painless and we encourage everyone aged between 16 and 24 who is sexually active to have a test at least once a year or whenever you have a new partner. Ask your doctor or nurse for more details or ask for a test pack at reception.

Several of our nurses are available to help patients choose the right contraceptive methods for their needs. In addition, we provide an Emergency Contraception service for any female patient.
Oral & Injectable Contraception: all nurses provide this service.
Coils (IUD/IUS) & The Contraception Implant : Please ask the receptionist which clinicians can offer this service for you
Counsellors & Youth Counsellor
Appointments by referral from your doctor. If you are between 12 and 24 you can call the Youth Counsellor directly on 0845 600 2516
Led by our nursing team, these appointments offer advice and health check-ups to patients diagnosed with diabetes. All patients are encouraged to attend for a full review once a year and have your HbA1C checked by a blood test six months after that

NHS Health Checks
Health checks are available to all patients aged 40 - 74 without a pre-existing condition, once every five years. For more details click here
Non NHS Examinations
The NHS does not cover non medical services such as insurance medicals, medicals for HGV licenses, fitness to undertake sports etc. We are happy to provide this service to all patients whether registered or not. There is, however, a fee payable in accordance with British Medical Association guidelines. Non NHS Fees
Orthopaedic Practitioners are experts in musculo-skeletal complaints. Employed by the Orthopaedic Department at Frimley Park Hospital, they hold daily clinics, mainly at Yateley Medical Centre but also at the other two surgeries. If you are 12 or over, you can book an appointment through the Help Hub
Patients can self-refer by calling 01483 782147 but eligibility criteria apply for on-going care.

A group meets at Yateley Medical Centre every Thursday evening where specially trained nurses give advice and support to patients trying to give up smoking.
